The Government of Philippines has announced and launched VaxCert.doh.gov.ph – VaxCertPH is the government’s vaccine certificate request website. People who have been fully vaccinated can use this online gateway to obtain a digital copy of their vaccine certificate as verification of their immunization status.

This digital vaccination certificate issued by the Department of Health (DOH) and the Bureau of Quarantine (BOQ) can be used on domestic and international travel.

Here is a step-by-step guide which will help you register online for VaxCert Certificate in 5 easy steps:

1. Visit the Official VaxCertPH website

Visit the www.VaxCert.doh.gov.ph. Below is the screenshot of the website for your reference.

Click on the “→ Continue Button” to request for a vaccine certificate.

2. Review and Accept Data Privacy

You will reach Vaccination Certificate Request page, Please review and accept the data privacy statement by ticking the box. Make sure you have your vaccination card and any government-issued ID with you before clicking Continue.

After reading the VaXCertPH Privacy Notice, click on tick box and click continue to proceed.

3. Answer the questions on screen and Enter Personal Information

You will be asked the following questions.

  • Were you vaccinated more than 48 hours ago for any of your doses?
  • Are you using this for International Travel?

Click the dropdown menu to choose your answer. After answering the questions, click “NEXT”.

Now enter your Personal Information and Vaccination Details such as Last Name, First Name, Middle Name, Suffix: (JR, SR, I, II, III, IV) and Birthdate (MM/DD/YYYY), 1st Dose Vaccination – Municipality/City of Vaccination, Vaccination Date (MM/DD/YYYY) etc…

After filling all the information, click on complete the reCAPTCHA verification checkbox and click on Next.

4. Review Information

Review all the information you entered. If all details are correct, click “Continue”, else, click “Go Back” if you need to change any information you entered.

After you click on continue, you will be able to see your vaccination details such as your full name, date of birth, vaccine name, and the your vaccination dates.

If you need to change any information click “Update record”, else click “Generate Vaccination Certificate”. If you wish to use it for your travel abroad, you may also enter your passport number before generating your vaccine certificate.

5. Save your VerCert Vaccination Certificate

Your vaccination certificate with QR code will be generated and displayed on the screen. After a successful generation, you may either:

  1. Download the VaxCertPH in PDF/JPG format.
  2. Print the Vaccination Certificate or
  3. take a screenshot of the VaxCert Certificate

VaxCert Registration offline Booth

Visit the official VaxCertPH booth of the local government unit (LGU) of your vaccination to generate your vaccine certificate. Locations of VaxCertPH Booths are as follows:

What is the difference between VaxCertPH and BOQ yellow card?

VaxCertPH Certifies user’s vaccination status in the Philippines against COVID-19.
BOQ yellow card Provides information on user’s vaccination status against diseases apart from COVID-19. Your destination country may have unique entrance requirements, such as VaxCertPH or the BOQ yellow card. Make sure you understand the requirements of your target country in order to travel smoothly.

Is it a requirement to have my VaxCertPH? What if I don’t have it yet?

For Domestic Use in the Philippines: Although VaxCertPH is NOT a requirement for purposes such as travel, school enrollment, employment, or any other transactions, you may need to present proof of your COVID-19 vaccination status when travelling domestically.

What if my VaxCert is not available?

If your VaxCertPH is not yet available, physical vaccination cards issued by the LGU or any proof of vaccination issued by the foreign country (if vaccinated abroad) can also be presented. In any case, we urge you to be aware of all requirements before traveling.
